HELD IN CITY HALL, 510 EAST FLORENCE BLVD., CONFERENCE ROOM A-2
OCTOBER 10, 2011 AT 10:00 A.M.


Chairman Jackson called this meeting to order at 10:00 a.m.

MINUTES:

Member Holiday moved to approve the minutes of May 2, 2011, meeting of the Casa Grande Fire Personnel Retirement Board. The motion was seconded by Member Leija and carried with a 3-0-voice vote.

SPECIAL BUSINESS:

Review and Accept Application for Survivor’s Benefit from Dorothea Ann Coxon (Deceased Member Nathan A. Coxon), and Approve Survivor’s Benefit
 
Nathan A. Coxon died on September 20, 2011.  He was receiving a monthly benefit of $3,120.73.  His wife, Dorothea Ann Coxon, submitted an application for Survivor’s Benefit.  Member Leija reported that Public Safety Personnel Retirement System’s representative have confirmed the survivor’s benefit of $2,496.58 to be correct.

Member Leija moved to accept an application for survivor’s benefit from Dorothea Ann Coxon, and to approve a monthly pension payable in the amount of $2,496.58 to Ms. Coxon, as the surviving spouse.  The motion was seconded by Member Holliday and carried with a 3-0 voice vote.

There being no further business for discussion, the meeting was adjourned at approximately
10:15 a.m.
